Police said the victim, 54-year old Roy Aguilar of Dallas, was impaled by some type of glass following the explosion, according to the report. RFAB is the world's first 300-mm analog fab, featuring 250,000 square feet of cleanroom space. Thursday's was in Bradford County. Today there was one in Lycoming County. State environmental officials said there was a malfunction at an Anadarko well pad north of Jersey Shore. Emergency crews responded along Dam Run Road at the Larry's Creek Fishing and Hunting Club a little after noon. Authorities said a gas production unit, something similar in size to a backyard shed, malfunctioned. Saturday afternoon dozens of customers and staff were overcome by the gas. "People with oxygen ad some people go inside the ambulance and they send them to the emergency room", said Merchant Inggrid None. Fifteen people were taken to the hospital for their exposure. None runs a nearby restaurant that had to be evacuated. One of her workers was among those who got sick. "He got dizzy and started feeling like shaky, and jittery so", None said. Workers at the salon called 911 after customers and staff started showing classic symptoms of carbon monoxide poisoning including the headaches and dizziness. "They weren't aware of what was causing it because carbon monoxide as you probably know is odorless, colorless, until you start to feel these symptoms" said Frisco Asst. Fire Chief Paul Siebert. Fire crews knew immediately what they had. A gas detector identified the carbon monoxide within seconds. "We were reading above 200 ppm which is a significant level for acute exposure which is short term exposure‰, Siebert said. (AP) - A sealed device used to check whether radiation detectors are working properly at Brookhaven National Laboratory leaked a small amount of radioactivity last month, lab authorities said. There was no threat to public health or the environment, but the lab has halted some operations while it investigates. The radioactivity was later found on two employees, in a parking lot and private vehicle, and in one of the facility's buildings, the lab said. The lab is reviewing policies, procedures and training programs. "Even the leak of a small amount of radiation is unacceptable," George Goode, assistant director for environmental safety and health at the lab on eastern Long Island, said Monday. "We are treating this very seriously." The lab announced in a statement Friday that it shut down all radiological operations following the Sept. 28 leak. The leak was first reported Monday by the Long Island Business News. Two employees were checking radiation detectors when a "sealed source" device containing a small amount of cesium-137 stored inside a lead container tipped over in the employees' pickup truck as they made their rounds, Goode said. Hazmat was called to the scene to deal with sulfur used in making fertilizer. Heavy smoke was reported at the scene and Marken Road was blocked off. Firefighters assessed the condition of the workers, decontaminated and treated them on scene, then sent them to ValleyCare Medical Center in Pleasanton as precaution. No patrons were affected, the fire department said. Fire department investigators determined the mixing of cleaning agents resulted in a hazardous gas. The owner of the building, Troy Hughes, and his son were both inside the building when the fire started, but the cause was undetermined. When the compound exploded, it caused minor cuts to Ibrahim's face, hands and lower forearms, he said. Ibrahim was taken to Shands, where he was treated for minor injuries, said Janine Sikes, director of UF public affairs. Other students were in the lab at the time of the explosion but did not feel the direct effects of the explosion. Judit Kovacs, a chemistry Ph.D. student, was three feet from the explosion when it happened. She said at first her ears were ringing and felt clogged. The contractor had the substance on his gloves, Graham said. The contractor's gloves and outer clothing was removed and the person was dry-decontaminated, Graham said. At approximately 7:50 this morning a semi tractor pulling a box trailer was northbound on I-65 near the 47 mile marker (south of U.S. 50-Seymour) caught fire due to overheated trailer brakes. The trailer was hauling 75 barrels of Xylene, which is a highly flammable and toxic liquid. Due to the fire and heavy smoke, both north and southbound I-65 lanes were shut down. The trailer fire began about 7:50 a.m. in the northbound lanes a couple of miles south of the U.S. 50 exit in Seymour, Indiana State Police Sgt. Noel Houze said. Slideshow: Crash Scene "The trailer was hauling a material called xylene, which is a highly flammable material," Houze said. The interstate was shut down in both directions for several hours, but southbound lanes reopened about 10:30 a.m. Northbound traffic was being taken off the interstate at exit 41 in Uniontown and directed to U.S. 31. "Northbound I-65 ∑ will remain closed for most of the day," Houze said. "The xylene has to be offloaded to another truck."
