An explosion of ammonia gas killed at least five workers at Ukrainian chemical maker PAT Concern Stirol, the prosecutor general?s office said.
The leak occurred at 2 p.m. today during maintenance in Horlivka, eastern Ukraine, where the plant is based, according to a statement released on the the website of the regional prosecutors office, which opened a criminal probe.

A 47-year-old man suffered burns to 30 per cent of his body after a chemical explosion at Deception Bay, north of Brisbane, on Tuesday.
Paramedics were called to the private residence in the semi-rural area of Priests Rd, Deception Bay after reports of a chemical explosion at 11.05am.
The site is less than 10km away from yesterday's blast which injured two. Monday's explosion came after a group of youths were allegedly making flamethrowers.
PRIESTS ROAD DECEPTION BAY
A spokeswoman for the Department of Community Safety said the man hurt in Tuesday's incident had been taken to Royal Brisbane Hospital and was in a stable condition.
"A 47-year-old man suffered serious burns to his arm, face, neck and chest," the spokeswoman said.
A spokesman for Queensland Police Service said the man was using a grinder to cut the top off a 200 litre drum that previously stored fuel when it exploded.

LANSING KROGER IN FRANDOR SHOPPING CENTER EVACUATED DUE TO HAZARDOUS MATERIAL

LANSING -- Authorities evacuated and closed a grocery store in Frandor Shopping Center Monday afternoon after a hazardous material sickened one person.
Lansing Fire Department public information officer Eric Weber said a 911 call was placed from Kroger, 443 Mall Court in Lansing, about 12:30 p.m. reporting a man was down inside the store. The first-arriving unit arrived at 12:31 p.m. and discovered the person "may have been overcome by something," Weber said.
Firefighters' air monitoring equipment tripped, indicating a hazardous substance was present. They treated the patient at the scene then transported him to Sparrow Hospital, Weber said. Firefighters then evacuated the building and reentered for further air monitoring and to locate the source.
Firefighters found the source of the hazardous material about 1:30 p.m., then isolated and ventilated the building to make it habitable for investigators, Weber said. The Lansing Fire Department was not disclosing the cause on Monday.
"It's under investigation," Weber said.
Corporate officials from Kroger have been involved in since the incident occurred, Weber said. The store remains closed until further notice.
"Kroger corporate has to work with the health department and MIOSHA, all those kinds of things, sot that the business can be opened back up to the public," Weber said.
The condition of the man sickened at the store was unknown as of Monday afternoon.

UNL BUILDINGS OPEN AFTER CREWS INVESTIGATE CHEMICAL ODOR

Buildings near the Scott Engineering Center were open Tuesday morning after Lincoln Fire and Rescue investigated a chemical odor on the University of Nebraska-Lincoln campus late Monday.
Police and fire officials were dispatched to the Scott building shortly after 5 p.m. on a report of a strong sulfur-like odor in the building. Officials asked those inside Scott, Nebraska and Othmer halls to vacate and restricted access to the buildings as they conducted floor-by-floor sweeps.
After firefighters were called out, UNL issued an alert by text message at about 7 p.m., encouraging people to avoid the area.
UNL?s Environmental Health and Safety department responded and ventilated the area believed to be the source of the odor, school officials said. The cause of the odor was not determined.
The buildings were re-opened and returned to normal operations at about 10:45 p.m. There were no injuries, officials said.
Officials conducted a follow-up sweep of the building early Tuesday morning and detected no hazardous odor.

A chemical scare at an East Tennessee restaurant raises questions about the consequences of mixing household chemicals.
The Texas Roadhouse in Alcoa reopened Saturday after the restaurant was evacuated Friday night.
An employee accidentally mixed chemicals and produced a dangerous gas cloud. Crews took 18 people, all restaurant employees, to Blount Memorial Hospital. They have been treated and released.
What happened at Texas Roadhouse is a common mistake seen in many homes.

MAKING THE RELATIONSHIP BETWEEN RISK ASSESSMENT AND RISK MANAGEMENT MORE INTIMATE

Keeping a separation between risk assessment and risk management is a common theme in environmental regulatory affairs. Values should play no part in the science of assessment, and management decisions often have to be based on more than the science and the values of scientists. Yet a couple of recent reports, from both the US (?Silver Book? (1)) and EU (Opinion of the Scientific Committees of the European Commission(2)) arenas, have drawn attention to shortcomings in the risk assessment process that derive, in part at least, from maintaining a too sharp separation between the assessors and the managers. Here we revisit the relationship between risk assessment and risk management with these developments as background and argue for a closer relationship. Our focus will be on the risks from chemicals in the environment but the principles will be more broadly applicable.

Paramedics treated 17 people and took 11 to hospitals on Monday when a safety valve released ammonia in the air at Schneider's Dairy in Whitehall.
No one had life-threatening injuries, said Allegheny County Department of Emergency Services Chief Alvin Henderson. Those transported to hospitals went as a precaution, said Whitehall police Chief Donald Dolfi.
Dairy employees reported the valve released about 50 pounds of ammonia, emergency officials said. The valve released the foul-smelling and -tasting gas when pressure in a tank rose to unsafe levels, Dolfi said.
The tanks, where ammonia is stored as a liquid, are designed to release the chemical when pressure rises to prevent an explosion.
?The system appeared to work as it was designed,? Dolfi said.

Seven employees have been injured at an explosion at Sun Chemical?s plant in East Rutherford, N.J, US.
The explosion occurred at a approximately 1.20 pm in the pre-mix room of the plant, according to a company statement. The East Rutherford Fire Department had the blaze under control by 1.45 however several neighbouring buildings were evacuated according to local media reports.

4 ARRESTED FOR ALLEGEDLY THROWING INCENDIARY DEVICES IN RIVERTON

RIVERTON ? City officials said Monday they are "saddened" by the weekend arrests of four teens, including the reigning Miss Riverton.
But in a brief statement, they declined to say much else, only noting: "Riverton City provides some sponsorship funding for the Miss Riverton Pageant. Riverton City does not recruit, select, or oversee pageant participants."
According to Capt. Clint Mecham, the teens were driving around the Riverton area, throwing the chemical reaction bombs at people and property in multiple locations.
A probable cause statement from the Salt Lake County Sheriff's Office states witnesses saw a gray or black vehicle with a dent on the front drive by a house in Riverton and saw four of the alleged incendiary devices thrown out of the vehicle. They also said they heard loud explosions.
Investigators from the Unified Fire Authority Arson/Bomb Squad found detonated plastic water bottles "with remnants of aluminum foil and a chemical substance" in the driveway and roadway of the site after 11 p.m. Friday, the statement read.
