DOW WORKER DEATH LIKELY DUE TO REACTION OF TRIMETHYLINDIUM WITH CLEANING FLUID

On Oct. 9, 2013, an explosion and fire at a Dow Chemical electronic materials facility in North Andover, Mass., led to the death of production operator Carlos A. Amaral, 51. According to a statement released by Dow and dated the end of January, the company?s investigation into the incident concluded that:

? An employee sustained injuries as a result of the overpressure of a small stainless steel manufacturing vessel during an operation associated with a Trimethylindium (TMI) manufacturing batch.
? An undesired and unexpected reactive chemical event occurred within the vessel as the employee was transporting the vessel from the glove box to the next manufacturing unit for further processing.
? The overpressure resulted in a release of reacted and unreacted materials and a fire.

The most highly probable cause of the unplanned event was the ingress of cleaning liquid from the cavity space of the ball valve into the crude TMI. Due to the nature of the event, it is impossible to completely validate this conclusion.

I asked Jeremy Cole, business communications manager at Dow Electronic Materials, whether Dow is changing anything regarding cleaning or other handling of the reaction vessel. He said yes, but so far he has declined to provide additional details.

The federal Occupational Safety & Health Administration is investigating the incident, because Massachusetts does not have its own occupational safety and health program. OSHA currently lists the case as open.

WOMAN IN CUSTODY AFTER DRUG LAB EXPLOSION AT DOWNTOWN HOTEL

SAN ANTONIO -- Police took a woman into custody and are searching for two men after a suspected drug lab explosion at a Downtown hotel Tuesday afternoon.

Emergency crews were called to the Days Inn at East Houston and Bowie Street shortly before 6 p.m. Tuesday.

According to a police department spokesperson, a woman inside a room at the hotel was creating some sort of drug and a chemical reaction occurred. Investigators are not sure at this point if it was methamphetamine or some other drug, but the chemical reaction blew out a window in the hotel room.

RESIDENTS EVACUATED DUE TO NATURAL GAS LEAK

Black Creek Drilling were drilling a natural gas well when the well lost pressure resulting in a well blow out.

Authorities are currently monitoring the air in area. There is a one and half mile of evacuated area that involves about 12 homes about 40 people. There are no reported injuries.

State police, hazmat, DEQ are on scene. Authorities are waiting for Wild Well Control out of Houston to arrive so a crew can go in and make an assessment.

Detour route coming in south of Eunice take LA 13 to LA 368 to LA 370 to LA 755 takes you into Eunice.

State Police say there is no shelter set up just yet for the people evacuated from their homes. But they are working with Red Cross to set one up.

Authorities are evacuating residents living within a mile and half of Highway 13 near Buckhorn in Acadia Parish.

Fire officials say a land rig was being drilled in the area and workers hit a gas pocket. As a result, natural gas is spewing out.

CHLORINE LEAK IN NORTH SCOTTSDALE; HAZMAT CREWS CALLED TO SCENE

SCOTTSDALE, Ariz. -- Hazardous materials crews were called out to a reported chlorine leak in north Scottsdale Tuesday night.
The first-alarm call came shortly before 10 p.m. near Hayden and Deer Valley roads.
The initial call was a report of a strange odor near the water treatment plant north of the Loop 101 and Pima Road.
Crews from Scottsdale, Phoenix and Tempe worked together to isolate the problem and determine any other needs.
No injuries were reported. No evacuations were planned.

CHEMICAL SPILL BADLY BURNS DRY-CLEANING EMPLOYEE

A dry-cleaning employee was severely burned by a chemical spill at the business Monday afternoon.

According to information form the Flagstaff Fire Department, firefighters responded to the business on the 1400 block of East Route 66 at about 4:30 p.m. on a report of a chemical spill. When crews arrived, they found that the employees had been evacuated from the business. One employee was suffering from severe burns and firefighters quickly decontaminated the victim and had the victim rushed to Flagstaff Medical Center. The victim was later flown to a burn center in Phoenix.

According to the subsequent investigation, although the chemical was a standard product used in dry cleaning, the chemical is heated to very high temperatures. That chemical came in contact with the employee, which caused the burns.

Fire officials would like to remind residents to be careful and safe and always read and follow directions when using any chemicals.

INSECTARY EQUIPMENT FIRE AT CORNELL

A fire caused by a mechanical failure on the roof of the University?s Insectary Laboratory was quickly extinguished by Ithaca firefighters Monday, according to an Ithaca Fire Department press release.

Thomas Basher, public information officer for the IFD, said the blaze was caused by the failure of a duct heater within the lab.

At approximately 11:45 a.m., Tompkins County 911 Dispatch received an alarm from the Cornell laboratory, according to the press release. Crews noticed smoke coming from an exhaust fan on the roof and found a fire in the roof assembly.

Though the building ? which is located on the eastern part of campus ? suffered ?moderate damage,? firefighters were able to quickly control the blaze by cutting roof material to gain access to the burning area, according to the press release. The fire was extinguished in approximately 90 minutes.

The IFD said that no one was injured in the fire.

HAZMAT CREW CALLED IN FOR WEST BRIDGEWATER CHEMICAL FIRE

The West Bridgewater Fire Department responded to a chemical fire at a metal processing plant Monday morning.
Fire Chief Leonard Hunt said the fire, at 340 Manley Street, began as an electrical fire that was inside of a tank full of chemicals. He did not know what the chemical was.
When they arrived there was smoke coming out of the front of the building. The fire was quickly knocked down.
The fire occurred at R&H Metal Processing, which, according to its website, does metal plating and x-ray florescence testing. An employee of the shop told fire officials that there was hydrochloric acid and nitric acid in the area of where the fire started.

FIRE AT IIT DELHI, NO INJURIES

A fire broke out in a chemistry laboratory of the Indian Institute of Technology in South Delhi on Monday morning. However, no injuries were reported in the incident.

According to fire officials, the blaze was reported from the chemistry laboratory adjacent to a hostel on the campus. ?We received a call at 11-25 a.m. that a fire had broken out in a high-rise building on the campus. Five fire tenders were pushed into service to douse the fire and the blaze was brought under control within an hour,? said a Fire Department official.

Fire officials said it was a low-density fire.

People in the vicinity of the campus said they heard the sound of a cylinder blast. Later it was found that the fire started from the chemistry lab because of some chemical reaction.

The building was immediately vacated and no one received injuries in the incident. Some of the laboratory apparatus was destroyed. The loss has not been estimated so far.
